Student-Athlete Forms

For your convenience we have provided many useful forms on one page.  Please see the Athletic Clearance tab for complete instructions. You can download the Transportation form and turn it in via email or in person. Email Ms. Ramirez (lramirez@bishopamat.org) or drop it off in the AD office.  The Physical form must be an original and turned in to the Athletic Department.

Transportation form

 The Transportation form is required for your student athlete in the case where the student will be transported to their sporting event by the school but will NOT be transported back to the school in the same manner. The form is required for every event based on one of the following circumstances:

1.  The parent/guardian will personally transport their son/daughter from Bishop Amat to or from the event destination.  Example: The student takes the bus to his/her game but the parent is going to take the student home from the game.

2.  The parent/guardian gives permission for their son/daughter to ride in the car of an approved parent driver. (Must have a current driver's license and insurance card on file with the athletic office.) 

3.  The parent/guardian gives permission for their son/daughter to drive himself to and/or from the event.  Students may not drive other students.

Transportation forms are due three (3) days prior to the event and must be filled out for every occurrence.

Physical Form

All students must have a current physical (valid for one year of date performed) on file in the Athletics Office. Physical Evaluation Forms are to be completed by a Medical Doctor (MD) or Doctor of Osteopathy (DO) ONLY. Physicals completed by ANY OTHER practitioner cannot be accepted, as is our school policy. Original must be submitted to the Athletics Office. 

Athletic Fees

All student athletes are required to pay an Athletic Fee. For your convenience you can pay the annual athletic fee of $250 online by clicking the link above to the right. The Athletic Fee is required only once during the academic school year regardless of the number of sports played. The link will open up after July 1st.
